Evan founded Form Health with the mission of making modern medical weight loss available for everyone. Evan is a member of the board of directors at Bicycle Health, the national leader in delivering medication assisted therapy to people with opioid dependency. He received a BA with honors at Dartmouth College, and an MBA with honors from the Harvard Business School.
